Medication record for PD
The combination of medications for Parkinson’s disease (PD) can become complicated, and filling in a medication record is an easy way of keeping you and your doctor up-to-date. The record can be used as a reminder to yourself, and is also useful to take along to medical appointments. It can also be carried around in a bag or pocket for use in emergencies, or when travelling abroad.
Follow the link below to download and print blank medication record tables, ready to fill in. Each table contains space for you to record your medications, and the instructions for taking them. Remember to include details of all the medications that you take – not just those that are taken for PD.
Don’t forget to update the details if your medication is changed at any point – start a new table so you can clearly see which medications you are to take each day.
